Behavioral Health Care Manager Responsible for assessing client needs, writing and monitoring care plans, and utilizing resources within guidelines.
Work involves case management, resource development and management, and community relations development.
Behavioral Health Address challenges faced by individuals with behavioral health related to social isolation Assess social determinants of health and transition between settings Assist with accessing services for individuals Assist ADRC staff with individuals in challenging situations Assist with training to enhance ADRC services with clients with behavioral issues Build relationships with CMHCs to expand and enhance access services for older adults and people with disabilities Case Management Conduct reassessments to determine client needs Develop care plans for assigned clients including informal supports Monitor care plans on the schedules indicated by funding sources Complete adult and child protective service referrals Adjust care plans based on reassessment results, available resources, client eligibility and family support Maintain client documentation and files according to program/agency standards Prepare daily, monthly, yearly scheduling system to monitor dates of assessments, reassessments, and updates Maintain strict confidentiality Strictly follow HIPAA guidelines Demonstrate knowledge of the interrelated functions of I&A services and case management Implement care management services timely for all client referrals Attend all scheduled training/meeting including state licensure/certification Assist with identifying participants in grant projects or special programs Keep up to date on new developments in the field of aging and governance Assist other ADRC departments as needed Resource Development/Management Accurately record time and tasks according to agency and funding source guidelines and time frames Display competence for all guidelines, rules/instructions for Medicaid Waiver, CHOICE, SSBG, Title III, Caregiver Promote private pay services for all programs sponsored by LifeStream Assist in training new care managers Share resource information with co-workers Make referrals to necessary agencies for services including durable medical equipment, mental health services and hospice Community Relations Develop and maintain working relationships with area organizations and service providers Demonstrate agency's customer service standards Conduct outreach activities for service referrals Participate in advocacy activities as assigned EDUCATION AND/OR EXPERIENCE Bachelor's degree in social work, sociology, counseling, psychology, gerontology, a valid IN RN license or a Bachelor's degree in any field with a minimum of two (2) years full-time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring; or a Master's degree in a related field may substitute for the required experience.
Must meet experience requirements outlined by the Division of Aging for A & D Medicaid Waiver, SSBG/Title III, CHOICE, and TBI Waiver.
LCSW is preferred.
CERTIFICATES, LICENSES, REGISTRATIONS Must meet case management certification requirements, and be approved for CHOICE, SSBG/Title III, A & D Medicaid Waiver and TBI.
Must possess a valid driver's license.
Must be covered, maintain coverage and provide documentation of current automobile insurance meeting LifeStream's amount requirement.
