This role involves assessing, planning, implementing, and evaluating care for individuals with dementia or other memory-related disorders.
The nurse works collaboratively with the interdisciplinary team to ensure the highest standard of care and quality of life for our residents as well as their families.
Responsibilities:
Assessment and Care Planning:
Conduct comprehensive assessments of residents' physical and cognitive health.
Develop and implement individualized care plans based on assessment findings.
Monitor and reassess residents regularly to adjust care plans as needed.
Medication Management:
Administer medications as prescribed and ensure residents' adherence to medication regimens.
Monitor for potential side effects and collaborate with healthcare providers to adjust medications when necessary.
Clinical Care:
Provide skilled nursing care, including wound care, injections, and other clinical procedures.
Monitor vital signs and assess overall health status regularly.
Dementia Care:
Implement specialized care techniques for individuals with dementia.
Foster a supportive and therapeutic environment for residents with memory-related disorders.
Behavioral Management:
Address and manage behavioral symptoms associated with dementia, such as aggression, agitation, or wandering.
Collaborate with the interdisciplinary team to develop and implement behavioral interventions.
Family and Caregiver Support:
Communicate effectively with residents' families and caregivers.
Provide education and emotional support to families coping with the challenges of memory-related disorders.
Documentation and Reporting:
Maintain accurate and up-to-date medical records for residents.
Communicate changes in residents' conditions to the interdisciplinary team.
Collaboration and Communication:
Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize resident care.
Communicate effectively with team members to ensure continuity of care.
Regulatory Compliance:
Adhere to all relevant healthcare regulations, including state licensing requirements and facility policies.
Qualifications:
Current LPN or RN licensure.
Experience in geriatric nursing, preferably in memory care.
Knowledge of dementia and Alzheimer's disease.
Strong communication and interpersonal skills.
Ability to work collaboratively in a team-oriented environment.
